Select an level:
When you are at the main menu, press Down, Down, Up, Down, R2, L2, Up, Down 
to select any desired levels.

Infinite arrows:
When you are at the main menu, press Down, Left, Up, Right, R2, L2, R2, L2 
for unlimited arrows.

Infinite health:
When you are at the main menu, press Up, Down, Up, Down, L1, L2, R2, R1 
for unlimited health.

Select an level:

Anti-gravity hair:

Unlock alternate totem:
