Giant head mode
Pause the game play and push L1 + L2 + R2 + Down.

All sphere mode
Pause the game play and push L1 + L2 + O + Square.

Stage select
Pause the game play and push R1 + Square, Triangle, O, X.

God mode
Pause the game play and push O + Square + Right, Left + R1 + R2. 
A sound of an explosion will confirm the correct code entry.

Unlock Cyborg Blake as playable charcter:
Pause the game play and push R1 + R2 + Triangle + O. 
If the code is accepted, you will hear "No Way!".
